Indications

Hypertension. Reduce risk of cardiovascular morbidity & mortality as measured by the combined incidence of cardiovascular death, stroke & MI in hypertensive patients w/ left ventricular hypertrophy. Treatment of diabetic nephropathy w/ elevated serum creatinine & proteinuria in Type 2 diabetics w/ history of hypertension.

Dosage

Hypertension 50 mg once daily. May be increased to 100 mg once daily if required. Patients w/ intravascular vol depletion 25 mg once daily. Reduction in the risk of cardiovascular morbidity & mortality in hypertensive patients w/ left ventricular hypertrophy Initially 50 mg once daily. A low dose of hydrochlorothiazide should be added &/or the dose of Cozaar should be increased to 100 mg once daily based on BP response. Renal protection in Type 2 diabetic patients w/ proteinuria & hypertension Initially 50 mg once daily. May be increased to 100 mg once daily based on BP response.

Contraindication

Pregnancy.

Precautions

Intravascular vol depletion, electrolyte imbalance, liver cirrhosis, hepatic or renal impairment, bilateral renal artery stenosis or stenosis of the artery to a solitary kidney. Lactation. Childn.

Adverse Reactions

Dizziness, asthenia, fatigue, vertigo, hypotension, hyperkalaemia, chest pain, oedema, palpitation, tachycardia, GI disturbances, back pain, muscle cramps, headache, insomnia, cough, nasal congestion, pharyngitis, sinus disorder, upper resp tract infection.

Drug Interaction

Concomitant use of K-sparing diuretics, K supplements/salt substitutes containing K may lead to hyperkalaemia. Effects may be attenuated by indomethacin.
