Indications  

Treatment of hypertension, for patients in whom combination therapy is appropriate. 

 

Dosage     

Fixed dose combination is not indicated for initial therapy. Usual starting & maintenance dose: 1 tab of Hyzaar once daily. May be increased to 2 tab of Hyzaar once daily or 1 tab of Fortzaar once daily. Max dose: 2 tab of Hyzaar once daily or 1 tab of Fortzaar once daily. Elderly No initial dosage adjustment necessary. Fortzaar should not be used as initial therapy in the elderly. 

 

Contraindication  

Hypersensitivity including hypersensitivity to other sulfonamide-derived drugs; anuria. When used during 2nd & 3rd trimesters, drugs that act directly on the renin-angiotensin system can cause foetal injury & death. Discontinue as soon as possible if pregnancy detected. 

 

Precautions  

Angioedema. Not for use in hepatic impairment or severe renal impairment (CrCl ≤ (smaller than or equal to) 30 mL/min). Hypotension, changes in renal function including renal failure, bilateral renal artery stenosis or stenosis of the artery to a solitary kidney, vol depletion, electrolyte imbalance. Thiazides may impair glucose tolerance, precipitate hyperuricemia &/or gout, increase cholesterol & triglyceride levels, decrease urinary Ca excretion, cause intermittent & slight increases in serum Ca, may also exacerbate or activate SLE. Not to be initiated in patients w/ intravascular vol-depletion. Lactation. Childn. 

 

Adverse Reactions   

Abdominal pain, asthenia, fatigue, chest pain, edema, swelling, palpitation, tachycardia, diarrhea, dyspepsia, nausea, back pain, muscle cramps, dizziness, headache, insomnia, cough, nasal congestion, pharyngitis, sinus disorder, upper resp infection, hypotension, hyperkalemia. Rarely, anaphylactic reactions, angioedema, vasculitis including henoch-schoenlein purpura, liver function abnormalities, anemia, myalgia, migraine, cough, urticaria, pruritus. 

 

Drug Interaction

Losartan: Rifampin, fluconazole, K-sparing diuretics, K supplements or salt substitutes containing K, NSAIDs. Hydrochlorothiazide: alcohol, barbiturates, narcotics, antidiabetic drugs, insulin, other antihypertensives, cholestyramine, colestipol, corticosteroids, ACTH, pressor amines, skeletal muscle relaxants, nondepolarizing neuromuscular blockers, lithium.
