Indications   

Treatment of hypertension, for patients in whom combination therapy is appropriate. 

 

Dosage   

Fixed dose combination is not indicated for initial therapy. Usual starting & maintenance dose: 1 tab once daily. May be increased to 2 tab once daily or 1 tab of Fortzaar once daily. Max: 2 tab once daily or 1 tab of Fortzaar once daily.

 

ADMINISTRATION:

May be taken with or without food. 

 

Contraindication     

Hypersensitivity including hypersensitivity to other sulfonamide-derived drugs; anuria. When used during 2nd & 3rd trimesters, drugs that act directly on the renin-angiotensin system can cause foetal injury & death. Discontinue as soon as possible if pregnancy detected. 

 

Precautions  

Angioedema. Not recommended for hepatic impairment or severe renal impairment (CrCl ≤ (smaller than or equal to) 30 mL/min). Hypotension & changes in renal function including renal failure. Bilateral renal artery stenosis or stenosis of the artery to a solitary kidney. Vol depletion or electrolyte imbalance. Thiazide therapy may impair glucose tolerance, precipitate hyperuricemia &/or gout, increase cholesterol & triglyceride levels, decrease urinary Ca excretion; intermittent & slight increases in serum Ca may occur. Hypersensitivity reaction may occur w/ or w/o a history of allergy or bronchial asthma. Thiazides have also been reported to exacerbate or activate SLE. Should not be initiated in patients w/ intravascular vol-depletion (eg. those treated w/ high dose diuretics). Lactation. Childn. 

 

Adverse Reactions  

Dizziness. Adverse experiences limited to those previously reported w/ losartan K &/or hydrochlorothiazide. Hyperkalemia, elevations in ALT. Anaphylactic reactions, angioedema, vasculitis including Henoch-Schonlein purpura (rarely), hepatits (rarely), diarrhea, cough, urticaria. 

 

Drug Interaction  

Losartan: Rifampin, fluconazole, K-sparing diuretics, K supplements or salt substitutes containing K, indomethacin. Hydrochlorothiazide: alcohol, barbiturates, narcotics, antidiabetic drugs (oral agents & insulin), other antihypertensives, cholestyramine, colestipol, corticosteroids, ACTH, pressor amines (eg. adrenaline), skeletal muscle relaxants, nondepolarizing (eg. tubocurarine), lithium, NSAIDs.
