Antiobesity Agents

Initially, 10 mg once daily. May increase to 15 mg once daily if response unsatisfactory (wt loss < 2 kg after 4 wk), provided 10 mg was well-tolerated.

May be taken with or without food.

Organic causes of obesity, history of major eating disorders, psychiatric illness, Gilles de la Tourette's syndrome, coronary artery disease, CHF, tachycardia, peripheral arterial occlusive disease, arrhythmia or cerebrovascular disease, inadequately controlled hypertension, hyperthyroidism, severe hepatic or renal impairment, benign prostatic hyperplasia w/ urinary retention, phaeochromocytoma, narrow angle glaucoma. Pregnancy & lactation. Childn ≤ (smaller than or equal to) 18 yr. Elderly > 65 yr. Concomitant or w/in 2 wk of MAOIs or other centrally-acting drugs for the treatment of psychiatric disorders or for wt reduction, or tryptophan for sleep disturbances. History of drug, medication or alcohol abuse.

Closely monitor BP & pulse rate. Epilepsy, mild to moderate hepatic or renal impairment, family history of motor & verbal tics. Women of childbearing potential should use adequate contraception.

Tachycardia, palpitations, hypertension, vasodilation, appetite loss, constipation, nausea, haemorrhoid aggravation, dry mouth, insomnia, dizziness, paraesthesia, headache, anxiety, sweating, dysgeusia.

Azole antifungals, macrolides, troleandomycin, cyclosporin, sumatriptan, dihydroergotamine, pentazocine, pethidine, fentanyl, dextromethorphan, SSRIs, drugs that increase BP or heart rate, MAOIs.
